After this session, the learner will be able to:


  • Identify the roles of the Advance Directive and the POST paradigm within the continuum of Advance Care Planning

  • Perform a structured, effective Goals of Care (GOC) Discussion with patients and family members utilizing a team-based approach

  • Ensure that the GOC are represented in the patient’s Advance Directive (if applicable)

  • Translate the patient’s GOC into a set of actionable, portable medical orders using the Virginia Physician Orders for Scope of Treatment (POST) Form (if applicable)

Respecting Choices’® Advanced Steps ACP Facilitator Certification is intended for healthcare clinicians (nurses, social workers, chaplains, physicians) and others involved in assisting those with serious illness or frailty in making treatment decisions based on their goals of care.

Purpose: To improve the effectiveness of the Advanced Steps ACP Facilitator Certification program through facilitation skills training.

Learning Outcomes

  1. Identify communication skills for facilitating Advanced Steps advance care planning (ACP) conversations.

  2. Create strategies to transfer individual goals, values, and preferences to a written plan, e.g., POLST program.

  3. Demonstrate beginning competency in facilitating Advanced Steps ACP conversations through role-play exercises.

  4. Become certified as an Advanced Steps ACP Facilitator through final competency role-play demonstration.


  • Prerequisite: Completion of the Respecting Choices® Online ACP Facilitator curriculum, a series of five interactive courses designed to prepare the participant for a robust classroom experience. The online prerequisite will take approximately five hours to complete. Access instructions will be emailed to you after payment for your training seat has been received.
